New Free Care Navigation Program Launched for LGBTQ+ Older Adults in the Coachella Valley

A new free resource is now available for aging members of the LGBTQ+ community in the Coachella Valley. Local nonprofit Triangle Care Management has launched a one-on-one care navigation program designed to connect LGBTQ+ older adults and people with disabilities with affirming health care providers and essential support services.

The program aims to reduce isolation while helping clients age safely, independently, and with dignity. Triangle Care Management says the initiative focuses on culturally appropriate care, long-term housing solutions through LGBTQ+-specific boarding care homes, and maintaining financial support resources for those in need.
